StartDate: 5/19/2025 Pay Rate: $2000.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans for the CPCU.

MINIMUM REQUIRED QUALIFICATIONS – KS/compact RN license; BLS; 2 years acute hospital case management experience; HCA experience

PREFERRED QUALIFICATIONS – Midas And HCA Case Management Experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – M-F 8-4:30

SYSTEMS – Meditech and Midas

START DATE – 5/19/2025
